Job Title: Direct Support Professional

Program: Residential Program

Location: Delaware, Bucks & Chester Counties

Schedule: Weekend Hours for Full-time and Part-time are available along with PRN Shifts.

Position Description: CADES is seeking enthusiastic individuals to work with medically fragile individuals with intellectual and physical disabilities in our residential facilities. DSPs provide necessary services to implement individual support plans and provide for personal care needs of individuals. Ideal candidates will be compassionate and caring with a willingness to ensure the house is run like your home. Responsibilities include, but are not limited to:

Responsibilities:

  • Personal care needs: bathing, toileting, feeding tubes, colostomy, and catheter care
  • Cleaning
  • Supporting meal preparation by cooking and feeding
  • Lifting and transferring
  • Working with individuals in wheelchairs
  • Properly uses and cares for all adaptive (feeding supplies, wheelchairs, walkers, etc) and medical equipment
  • Assistance with activities of daily living (ADL), including bathing, personal care, lifting, and transferring
  • Administering medication
  • Complete documentation for individuals as per agency policy
  • Community outings and doctor appointments
  • Budgeting consumer and program funds
  • Shopping for consumer and program needs
  • Providing transportation using agency vehicles

Required Knowledge/Qualifications:·

  • Valid Driver’s License for the state of residence with a clean driving record and minimum three-year driving history
  • Current PA State Police & FBI clearance for anyone living outside PA in the past two years.
  • High School diploma or GED
  • Direct support experience preferred
  • At least 21 years of age
  • Basic computer skills
  • Willingness to drive larger vehicles

    Must be available to work evenings, weekends, and holidays.

Job Title: Medical Appointment Representative

Program: Residential Program

Schedule: Full-time – Monday through Friday – 40 hours

Position Description: CADES is seeking a Medical Appointment Representative to promote the delivery of quality services to individuals residing in the CLA Program. The position is to provide transportation to and from medical appointments, advocacy with community caregivers and support with scheduling, documentation, and filing for CLA residents.

Responsibilities:

  • Works directly as a team with the CLA nursing team and operations manager to coordinate schedule and manage medical appointments and calendar for 104 CLA residents.
  • Provide transportation for CLA residents to and from appointments.
  • Collects pertinent paperwork, insurance cards, and any medical history; and gains understanding of the purpose of the medical visit, questions, and points for advocacy from the nursing team prior to medical appointments.
  • Ensures all paperwork documentation is completed properly and thoroughly during visit, including but not limited to, CADES appointment forms, physical forms, prescriptions for medication and DME (Durable Medical Equipment), referrals and other related documents as required/requested.
  • Communicates directly with the Health Care Coordinator/nursing team regarding the results of the visit and required follow-up activities.
  • Schedules follow-up appointments at time of visit as applicable.
  • Ensures prescriptions are handed off appropriately for follow-up immediately following visits.
  • Files appointment paperwork in a timely manner.
  • Supports the timely distribution & procurement of appointment paperwork for individuals supported on appointments by Direct Support Professionals.
  • Coordinates schedule with the Operations Manager to maximize availability in supporting the CLA needs.

Qualifications:

  • Associates Degree preferred. High School diploma and 2 years’ experience working in a residential program required.
  • Valid Driver’s License for state of residence with a clean driving record and minimum three year driving history
  • Current PA State Police clearance; FBI clearance for anyone living outside PA in the past two years.

Job Title: Operations Manager

Program: Residential Program

Schedule: Full-time – exempt

Position Description: The Operations Manager purpose/s to promote the delivery of quality services to individuals residing in the CLA Program. The Operations Manager ensures compliance with all CADES policies and procedures, state regulations, accrediting measures and insurance standards. Responsibilities include the following:

Medical Appointment:

  • Strategically plan appointment schedule and assignments in order to maximize resources and meet programmatic goals. 
  • Coordinate copays and communicate to the team in advance of the appointment. 
  • Communicate appointment schedules.
  • Analyze, plan and prevent missed medical appointments.
  • Provide direct supervision and coaching to the Medical Appointment Representative (MAR).

Vehicle Monitoring & Oversight:

  • Coordinate the use of agency vehicles within the home.
  • Manage vehicle placement at the homes to ensure the vehicle fits the needs of the program.
  • Coordinate routine vehicle audits to ensure cleanliness; monitor for any damages or maintenance needs; ensure safety features are present and functional including: tie-downs, seatbelts, fire extinguishers, and first aid kits.
  • Coordinate with Staff Development to complete employee training on driving and vehicle tie-downs.
  • Monitor driver behavior through reports and monitoring including

Billing & Authorizations:

  • Monitor approved units in individual’s ISP ensuring adequate available units for billing.
  • Collaborate with the fiscal department on billing issues.
  • Complete justifications for the Needs Exception Allowance Tool.
  • Submit requests for Supplemental Habilitation and SIS assessments and follow up proactively until resolution.
  • Attend CLA Finance Committee biweekly to report out on billing issues and requests.

Required Knowledge/Qualifications:

  • Bachelor’s degree preferred
  • Minimum of two years’ experience working in a residential program preferred.
  • Current PA State Police & Child Abuse clearances; FBI clearance for anyone living outside PA in the past two years. 
  • Valid driver’s license and 3-year adequate driving record; maintain adequate vehicle liability insurance.

Job Title: Residential Program Manager

Program: Residential Program

Location: 401 Rutgers Avenue, Swarthmore, PA

Job Description: CADES is currently seeking a full-time Program Manager to work in our CLA program in the Delaware County area. The Program Manager will oversee and ensure completion of all required program planning for individuals. Responsibilities for this position include:

Program Oversight:

  • Actively participate in all programmatic meetings including ISP & Team Meetings, monthly Med Meetings, Behavior Management meetings & Collaborative Care meetings
  • Role model effective communication & problem-solving skills
  • Actively participate in the Incident Management process
  • complete vehicle oversight & coordination.
  • Participate in the admissions process for all referrals
  • Participate in scheduled monitorings with Supports Coordinators & County entities
  • In conjunction with Program Specialist, act as a liaison for Health Care Coordinator, Behavior Support Specialist, Operations Manager, Trainer & Administrative Coordinator for all homes, individuals & staff under the assigned area of oversight
  • Coordinate the medical needs of the individuals
  • Participate in on-call responsibilities based on the needs of the program

Supervision:

  • Direct supervision of House Supervisors; role model effective supervision strategies; work with the HS to interview & recommend for hire Direct Support Professionals
  • Manage to the ISP to ensure implementation & programming is completed.
  • Complete Performance Evaluations for House Supervisors; work with HS to complete Performance Evaluation for Direct Support Professionals
  • Screen available candidates, conduct on-site interviews & make recommendations for hiring. Complete post-orientation onboarding including: site-specific orientation; vehicle & driver’s training; Medication Administration & MAR reviews.
  • Ensures timely, comprehensive & effective communication to staff to ensure the integrity of all program requirements. Conducts house meetings with assistance from the Program Specialist
  • Review and approve timesheets & time off requests for House Supervisors & DSPs
  • Review and analyze documentation to ensure compliance.
  • Ensure staff working in the home are trained & understand the ISP for all individuals in the home. Complete ISP training for all staff coverage, transfers & staffing changes.
  • Complete audits & ensure compliance of health and safety of individuals
  • Monitor and oversee medication administration including
  • Other duties as assigned based on programmatic needs

Requirements include:

  • Bachelor’s degree with 2 years experience working directly with individuals with disabilities or Associate’s degree (or equivalent) with 4 years experience working directly with individuals with disabilities
  • Current PA State Police & Child Abuse clearances; FBI clearance for anyone living outside PA in the past two years
  • Valid driver’s license and adequate 3-year driving record; maintain adequate vehicle liability insurance
  • Knowledge of PA Department of Public Welfare 6400 regulations
