Job Title: Direct Support Professional

Program: Residential Program

Location: Delaware, Bucks & Chester Counties

Schedule: Weekend Hours for Full-time and Part-time are available along with PRN Shifts.

Position Description: CADES is seeking enthusiastic individuals to work with medically fragile individuals with intellectual and physical disabilities in our residential facilities. DSPs provide necessary services to implement individual support plans and provide for personal care needs of individuals. Ideal candidates will be compassionate and caring with a willingness to ensure the house is run like your home. Responsibilities include, but are not limited to:

Responsibilities:

  • Personal care needs: bathing, toileting, feeding tubes, colostomy, and catheter care
  • Cleaning
  • Supporting meal preparation by cooking and feeding
  • Lifting and transferring
  • Working with individuals in wheelchairs
  • Properly uses and cares for all adaptive (feeding supplies, wheelchairs, walkers, etc) and medical equipment
  • Assistance with activities of daily living (ADL), including bathing, personal care, lifting, and transferring
  • Administering medication
  • Complete documentation for individuals as per agency policy
  • Community outings and doctor appointments
  • Budgeting consumer and program funds
  • Shopping for consumer and program needs
  • Providing transportation using agency vehicles

Required Knowledge/Qualifications:·

  • Valid Driver’s License for the state of residence with a clean driving record and minimum three-year driving history
  • Current PA State Police & FBI clearance for anyone living outside PA in the past two years.
  • High School diploma or GED
  • Direct support experience preferred
  • At least 21 years of age
  • Basic computer skills
  • Willingness to drive larger vehicles

    Must be available to work evenings, weekends, and holidays.

Job Title:  Residential Program Specialist

Program: Residential Program

Location: 401 Rutgers Avenue, Swarthmore, PA

Job Description: CADES is currently seeking a full-time Program Specialist to work in our CLA program. CADES maintains residential facilities in Delaware, Chester, and Bucks County. The Program Specialist will oversee and ensure the completion of all required program planning for individuals. Responsibilities for this position include:

Program Oversight:

  • Completion & oversight of implementation of all program planning within the guidelines of regulatory standards including; assessments; ISPs; monthlies; quarterlies; plans of support.
  • Role model effective communication & problem-solving skills.
  • Monitor all needed DME for individuals including; ensure equipment listed in ISP is present & functioning; training material is available in the home; equipment is clean & well maintained; requests for repairs & new equipment are submitted & follow-up is completed.
  • Point person for Admissions Coordinator throughout the referral process.
  • Actively participate in the Incident Management process
  • Oversee & manage regulatory monitoring for individuals
  • Liaison for communicating and updating the team on the needs of the individuals
  • In conjunction with Program Manager, act as the liaison for Health Care Coordinator, Operations Manager, Trainer & Administrative Coordinator for all homes, individuals & staff under the assigned area of oversight
  • Manage individual financials ensuring completion of spend downs, timely completion of contracts, timely payments of bills and monthly account audits.
  • Coordinate scheduled monitoring with the Support Coordinators & County entities.
  • Monitor monthly attendance for individuals & submit to fiscal department for billing
  • Monitor & oversee ISP implementation within the home. Ensure everyone in the home aligns to the ISP.
  • Proactively & effectively communicate with individual’s families & teams, ensuring accurate & timely communication.

Supervision:

  • Ensure timely, comprehensive & effective communication to staff to ensure the integrity of all program requirements. Conducts house meetings with assistance from Program Manager
  • Participate & attend all mandatory meetings necessary and related to role
  • Review and analyze documentation to ensure compliance. Complete staff retraining as needed for concerns
  • Complete audits & ensure compliance of health and safety of individual clients, required documentation, physical plant, vehicle maintenance, and staff safety requirements
  • Monitor and oversee medication administration

QUALIFICATIONS

Required Knowledge, Skills, and Abilities:

  • Bachelor’s degree with 2 years experience working directly with individuals with disabilities or Associate’s degree (or equivalent) with 4 years experience working directly with individuals with disabilities
  • Current PA State Police & Child Abuse clearances; FBI clearance for anyone living outside PA in the past two years
  • Valid driver’s license and adequate 3-year driving record; maintain adequate vehicle liability insurance
  • Knowledge of PA Department of Public Welfare 6400 regulations
