Job Title: Direct Support Professional

Program: Residential Program

Location: Delaware, Bucks & Chester Counties

Schedule: Weekend Hours for Full-time and Part-time are available along with PRN Shifts.

Position Description: CADES is seeking enthusiastic individuals to work with medically fragile individuals with intellectual and physical disabilities in our residential facilities. DSPs provide necessary services to implement individual support plans and provide for personal care needs of individuals. Ideal candidates will be compassionate and caring with a willingness to ensure the house is run like your home. Responsibilities include, but are not limited to:

Responsibilities:

  • Personal care needs: bathing, toileting, feeding tubes, colostomy, and catheter care
  • Cleaning
  • Supporting meal preparation by cooking and feeding
  • Lifting and transferring
  • Working with individuals in wheelchairs
  • Properly uses and cares for all adaptive (feeding supplies, wheelchairs, walkers, etc) and medical equipment
  • Assistance with activities of daily living (ADL), including bathing, personal care, lifting, and transferring
  • Administering medication
  • Complete documentation for individuals as per agency policy
  • Community outings and doctor appointments
  • Budgeting consumer and program funds
  • Shopping for consumer and program needs
  • Providing transportation using agency vehicles

Required Knowledge/Qualifications:·

  • Valid Driver’s License for the state of residence with a clean driving record and minimum three-year driving history
  • Current PA State Police & FBI clearance for anyone living outside PA in the past two years.
  • High School diploma or GED
  • Direct support experience preferred
  • At least 21 years of age
  • Basic computer skills
  • Willingness to drive larger vehicles

    Must be available to work evenings, weekends, and holidays.

Job Title: Residential Program Manager

Program: Residential Program

Location: 401 Rutgers Avenue, Swarthmore, PA

Job Description: CADES is currently seeking a full-time Program Manager to work in our CLA program in the Delaware County area. The Program Manager will oversee and ensure completion of all required program planning for individuals. Responsibilities for this position include:

Program Oversight:

  • Actively participate in all programmatic meetings including ISP & Team Meetings, monthly Med Meetings, Behavior Management meetings & Collaborative Care meetings
  • Role model effective communication & problem-solving skills
  • Actively participate in the Incident Management process
  • complete vehicle oversight & coordination.
  • Participate in the admissions process for all referrals
  • Participate in scheduled monitoring with Supports Coordinators & County entities
  • In conjunction with Program Specialist, act as a liaison for Health Care Coordinator, Behavior Support Specialist, Operations Manager, Trainer & Administrative Coordinator for all homes, individuals & staff under the assigned area of oversight
  • Coordinate the medical needs of the individuals
  • Participate in on-call responsibilities based on the needs of the program

Supervision:

  • Direct supervision of House Supervisors; role model effective supervision strategies; work with the HS to interview & recommend for hire Direct Support Professionals
  • Manage to the ISP to ensure implementation & programming is completed.
  • Complete Performance Evaluations for House Supervisors; work with HS to complete Performance Evaluation for Direct Support Professionals
  • Screen available candidates, conduct on-site interviews & make recommendations for hiring. Complete post-orientation onboarding including site-specific orientation; vehicle & driver’s training; Medication Administration & MAR reviews.
  • Ensures timely, comprehensive & effective communication to staff to ensure the integrity of all program requirements. Conducts house meetings with assistance from the Program Specialist
  • Review and approve timesheets & time off requests for House Supervisors & DSPs
  • Review and analyze documentation to ensure compliance.
  • Ensure staff working in the home are trained & understand the ISP for all individuals in the home. Complete ISP training for all staff coverage, transfers & staffing changes.
  • Complete audits & ensure compliance of health and safety of individuals
  • Monitor and oversee medication administration including
  • Other duties as assigned based on programmatic needs

Requirements include:

  • Bachelor’s degree with 2 years’ experience working directly with individuals with disabilities or Associate’s degree (or equivalent) with 4 years’ experience working directly with individuals with disabilities
  • Current PA State Police & Child Abuse clearances; FBI clearance for anyone living outside PA in the past two years
  • Valid driver’s license and adequate 3-year driving record; maintain adequate vehicle liability insurance
  • Knowledge of PA Department of Public Welfare 6400 regulations

Job Title: CLA Executive Director

Program: Residential Program

Schedule: Full-time – Exempt

Position Description: The purpose/s of this position is to monitor and promote the delivery of quality services to programs serving individuals in a community-based homes. The CLA Executive Director sets the vision and operational objectives to align with CADES strategy and vision to be a provider and employer of choice. The Executive Director provides oversight, direction and strategy to effectively manage the budget, maximize positive operating margin and recommend investments to meet organizational objectives.

Duties and Responsibilities:

Plan the program –

  • Plan the delivery of the overall program and its activities in alignment with leading trends in best-practices such as person-centered planning, self-determination and ODP Every Day Lives.
  • Align new initiatives to support the strategic direction of the organization.
  • Develop and implement long-term goals and objectives to achieve the successful outcomes for each program.
  • Implement a program evaluation framework to assess the strengths of the program and to identify areas for improvement

Lead the program –

  • Ensure that program activities operate within the policies and procedures of the organization and comply with all relevant legislation and professional standards.
  • Utilizes varied data collection techniques to analyze impact and implementation integrity of policies, procedures and standard practices related to individual care and programming across departments. Provide direction to capitalize on areas of strength and address areas of weakness.
  • Lead an organizational structure that encourages teamwork, leadership, ownership and accountability in all roles throughout the program.
  • Effectively communicate with clients and other stakeholders to gain community support for the program and to solicit input to improve the program.
  • Ensure exceptional stakeholder satisfaction. Role models excellence in demonstrating behaviors that instill respect, trust and growth among all constituents.
  • Liaise with other Directors to ensure the effective and efficient program delivery

Staff the program –

  • Utilizes varied data collection techniques to analyze impact and implementation integrity of policies, procedures and standard practices related to human resources in the department.
  • Foster an environment across the CLA and intersecting departments that upholds a philosophy of positive teamwork.
  • Collaborate to implement practices that prioritize recruitment, selection, training and retention activities of a professional, diverse, well-qualified program staff, adjusting to trends in the internal and external environment.
  • Upholds a performance management process for all program staff that includes consistent feedback for positive displays of behavior as well as constructive feedback aligned with the organization’s progressive discipline policy.

Control the program –
a) Fiscal

  • Collaborate with the fiscal team to manage the fiscal performance of the department.
  • Develop the annual budget with the fiscal team and in coordination with ancillary departments (operations, facilities, fleet, training, nursing).
  • Identify and manage to key performance indicators to ensure the program operates within the approved budget
  • Plan and monitor budgeted program expenditures
  • Evaluate opportunities and threats to meeting annual and future fiscal targets
  • Monitor fidelity of fiscal responsibilities according to regulations. Communicate and share regulatory changes with the fiscal team.

b) Risk

  • Identify and evaluate the risks associated with program activities and take appropriate action to control the risks
  • Monitor the program activities on a regular basis and conduct an annual evaluation according to the program evaluation framework
  • Ensures program compliance through auditing procedures in relation to CADES policy and procedures, government regulations, insurance and any related accrediting measures.
  • Accountable survey processes through consistent program preparation and monitoring, and ensuring generalization fidelity of action plans across the department.
  • Plan, implement and generalize comprehensive quality improvement plans, including data collection, analysis and reporting, that are based on best practices or areas identified for improvement.
  • Monitor and report on programmatic, health and safety standards within the division and follow up on areas needing improvement.

c) Communicate

  • Lead through oral and written communication that demonstrates the values of the organization including respect, trust and growth.
  • Write effective reports on the program to meet the lens of specific stakeholders
  • Communicate to internal and external stakeholders in a manner that is both informative and inclusive
  • Ensures consistency in written documentation and record retention that align with CADES policy/expectations to record procedures, how-to guides, training, meeting actions and operational guidance.

Continually improve the program –

  • Evaluate external trends, opportunities and risks, as well as internal strengths, weaknesses and fidelity; propose and create plans to ensure efficiency in practices and effective outcomes.
  • Create short and long term objectives to remain current with trends and policies, look to create a unique niche and remain competitive
  • Look for and create opportunities to advance the mission through new program offerings
  • Acquire and act upon customer feedback

Qualifications:

  • Master’s Degree preferred, Bachelor’s Degree required in related field.
  • Minimum of three years’ experience working in with individuals with disabilities and/or chronic illness.
  • Minimum of five years’ experience providing supervision to individuals in a management role, and at least two years providing supervision to individuals in a director role.
  • Current PA State Police & Child Abuse clearances; FBI clearance for anyone living outside of PA in the past two years.
  • Valid driver’s license and 3-year adequate driving record; maintain adequate vehicle liability insurance.
