Job Title: Direct Support Professional
Program: Residential Program
Location: Delaware, Bucks & Chester Counties
Position Description: CADES is seeking enthusiastic individuals to work with individuals with intellectual and physical disabilities in our residential facilities. Ideal candidates will be compassionate and caring with a willingness to ensure the house is run like your home. Responsibilities include, but are not limited to:

  • Cooking
  • Cleaning
  • Assistance with activities of daily living (ADL), including bathing
  • Administering medication
  • Community outings and doctor appointments
  • Budgeting consumer and program funds
  • Shopping for consumer and program needs
  • Providing transportation using agency vehicles

Hours: Evenings, weekends & overnight shifts

Qualifications:

  • Valid Driver’s License for state of residence with a clean driving record and minimum three year driving history
  • High School diploma or GED
  • Direct support experience preferred
  • At least 21 years of age
  • Basic computer skills
  • Willingness to drive larger vehicles

Job Description

CADES is seeking an LPN to support our residential program in Delaware, Chester and Bucks County working with adults with disabilities. Responsibilities include overseeing and managing the medical needs of the individuals within the residential facilities. Position works daytime hours but must be available for flexible hours including evenings and weekends for on-call responsibilities.

Essential duties of the position include:

  • Maintain up to date knowledge about consolidated waiver and PA Department of Public Welfare 6400 regulations.
  • Function as a member of the inter-disciplinary team. Attend all team meetings regarding consumers.
  • Effectively communicate with and maintain a conducive relationship with other individuals involved in the care of each consumer.
  • Meet monthly with Program Managers to discuss the overall effectiveness of health care services.
  • Develop health care plan addressing health concerns of each consumer which complies with state & county guidelines and recommendations. Update regularly as health concerns arise or needs change.
  • Write medical history of each consumer which complies with 6400 regulations and update regularly as health concerns arise or needs change.
  • Review the consumer’s individual health status, including dietary needs on an ongoing basis.
  • Train and educate staff persons regarding the needs of the consumer per agency policies and procedures.
  • Respond to emergencies involving the consumers’ health and safety and intervene.
  • Analyze, interpret and summarize medical records and date. Review all medical visit reports, physical forms, lab work/test results, medication logs and prescriptions to ensure that content and timeliness are in compliance with 6400 regulations.
  • Coordinate all hospital discharges. Complete all discharge materials, reports, and follow-ups.
  • Interface with pharmacy for all medication concerns.

Qualified candidates will possess the following skills:

  • Practical nurse, currently licensed in the Commonwealth of Pennsylvania
  • Clear Criminal History and Child Abuse checks. FBI check as required by regulations
  • Valid Driver’s License and clear three year driving history
  • Maintain adequate auto liability insurance

The positions requires candidate to work weekends on a rotation with on call responsibilities. Houses are located in Delaware, Chester & Bucks counties.
