Job Title: Quality Assurance Coordinator

Position Description: The Quality Assurance Coordinator is a full-time non-exempt position with the purpose/s to monitor and promote the delivery of quality services to students and individuals with internal and external criteria, policies and procedures, regulations, standards, and best practices. Position will coordinate and assist with efforts of development and implementation of a comprehensive quality improvement plan. Coordinate and assist with designing, developing, facilitating, and implementing program policies and procedures. Monitor and promote the safety and health of all individuals.

Responsibilities:

  • Leads the prevention and analysis of safety and quality assurance initiatives for the organization.
  • Leads in the collection, analysis, and presentation of the organizational balanced scorecard data.
  • Assist in the development and implementation of the Quality Management Plan.
  • Maintain up to date knowledge about Pennsylvania Department of Human Services Enterprise Incident Management (EIM) System and current standards.
  • Conduct investigations and update EIM system with required information for individuals served.
  • Maintain current Pennsylvania Department of Human Services Certified Investigator and Quality Management credentials.
  • Represent CADES during County and/or Adult Protective Services investigations.
  • Represent CADES during required unemployment hearings as related to investigations.
  • Ensure a safe and secure environment for individuals, self, and co-workers by exceeding expectations of minimum regulations to determine root cause and recommend action plans.
  • Work with Director of Facilities and Transportation and their staff to ensure that the facility and vehicles are maintained and operated in a safe, secure, and lawful manner and within all appropriate Program regulations.
  • Lead the Certified Investigator Peer Review (CIPR) program (quarterly).
  • Assist and assess monthly drills including, fire, intruder, silent alarm, and hazardous weather.
  • Serve on the Safety Committee which provides an avenue for all employees to contribute to workplace safety.
  • Review incidents and injuries to identify root causes and plan ways to prevent future incidents and injuries.
  • Participate in monthly Safety Committee meetings.
  • Complete annual safety committee training for membership.
  • Participate as critical team member during facility incidents.
  • Maintains required certifications.
  • Participate on committees as requested including Certified Investigation Peer Review Committee, Quality Assurance Committee and Human Rights Team.
  • Manage the Human Rights Team and maintain all documentation in accordance with current regulations.

QUALIFICATIONS:

  • Bachelor’s degree required; Master’s degree preferred.
  • A minimum of 2 years’ experience working directly with individuals with disabilities preferred.
  • Current PA State Police & Child Abuse clearances; FBI clearance for anyone living outside PA in the past two years.

Position Title:          Assistant Director of Leadership Development

FLSA Classification:    Exempt

SUMMARY:

The Assistant Director of Leadership Development is a full-time exempt position with the purpose to deliver quality education and skill development that aligns with best practices and leadership standards to new and existing employees that are in a managerial role. The Assistant Director of Leadership Development is responsible to come alongside the employee to teach, model, observe, coach, provide feedback and assess skills. The AD plans, coordinates, delivers training and ongoing analysis of individual employee and cohort growth needs.  Position requires a flexible schedule to meet the various needs of the programs. 

DUTIES AND RESPONSIBILITIES:

Professional Development

  • Ensures the integrity of the Management orientation process (training content, structure, delivery, skill competence) is trained out consistently in alignment with agreed upon procedures
  • Provide management and leadership training and professional development activities with the goal of ensuring proper knowledge and skills for management job roles, and meeting recruitment and retention goals by creating a positive management employee experience.
    • Co-create Advancement of Skills Plans for managers that aligns the skill sets required for their role and the growth strategy to meet the objectives.      
    • Supports the onboarding of managers and supervisors, coordinating and providing education and coaching on the skills necessary to complete their job-specific responsibilities.
    • Provides on-going coaching and support to managers intermittently as they advance in their personal skill plan, request coaching support and on an as-needed basis.
    • Research, adopt and develop training modules, and write accompanying training plans.
    • Educate managers as “trained trainers” applying best practices in demonstrating cultural competence, adult learning theory and the training industry.
      • Role model best practices in training and coaching sessions with managers. Demonstrates how skills are learned, practiced, and then generalized to the natural setting.
      • Utilizing the “I Do, We Do, You Do” training technique, coach managers to become trained coaches that ensure staff competence and confidence in skill application by coming alongside employees and coaching them until proficiency is demonstrated, and on a maintenance schedule thereafter.
    • Coach managerial staff by coming alongside of them in the field to teach, model, provide feedback and assess skill level, to help the staff members grow and improve.
      • The coach will provide education, skill training and assess the level of mastery of skills in the training room and in the field.
      • Provides regular communication with the direct supervisors of the managers.
      • Conduct skill assessments and observations of staff to ensure comprehension of material.

Team Liaison

  • Liaison to Human Resources, Director(s)/Assistant Directors(s) of programs, Nursing Department and Quality Assurance department to ensure consistency in the learning process for managers.
    • Collaborates with the nursing department to assist with joint training and skill assessment for management staff.
    • Partner with the program director(s)/assistant director(s) to develop training goals and learning plans for group learning. 
    • Provides input, observation, and feedback on onboarding guides and training for manager roles.
    • Evaluates data and provides input for training topics to best meet the ongoing needs of specific programs or organization as a whole.
    • Listens for opportunities to improve the manager employee experience by connecting with teams to understand where opportunities for learning can improve outcomes.
    • Encourages teamwork and relationship development between departments to grow the manager networks of support.    
  • Supports managers of programs to ensure regulatory compliance with training.
    • Maintains all documents and records related to training completion and regulations.
    • Analyzes and shares data in a manner that keeps key stakeholders informed and encourages personal responsibility to meet stated goals.
    • Liaison with Assistant Director(s) to support manager and DSP success to meet the required documentation standards, mandates and hours of annual training as stated in the regulations and CADES policies.
    • Assists with assessments of medication administration and observations of staff, in conjunction with CLA Leadership.
  • Utilizes, models, and generalizes learning from the Leadership Institute course curriculums.
  • Creates opportunities to reinforce confidentiality policies regarding individuals served and employees.
  • Capitalizes on opportunities to model and coach around areas of emotional intelligence, self-reflection, and self-management.
  • Obtain and maintain proficient knowledge and skills incorporated in management orientation in order to observe, coach and provide feedback to managers as trained trainers. Examples of skills can include: Risk management, Clinical skills, Conflict resolution skills

Qualifications

  • Bachelor’s Degree in human services related field.
  • Experience providing training and/or education; experience in coaching preferred.
  • A minimum of 3 years’ of demonstrated success providing supervision and feedback to direct reports.
  • A minimum of 2 years’ experience working with individuals living with multiple disabilities, including a strong understanding of the safety and communication support required for individuals with physical disabilities.
  • Current PA State Police & Child Abuse clearances; FBI clearance for anyone living outside of PA in the past two years.
  • Valid driver’s license and 3-year adequate driving record; maintain adequate vehicle liability insurance.
  • Ability to attend meetings off site using personal transportation.
  • Must be available for evening and weekend work.

Position Title: Human Resources Director

Location: 401 Rutgers Avenue, Swarthmore, PA 19081

SUMMARY: The Human Resources Director is a full-time exempt position with the purpose/s to implement and promote the delivery of employment services to meet the mission and vision of CADES as an organization. This is accomplished by ensuring compliance with all CADES policies and procedures, state regulations, accrediting measures, and insurance standards. 

Duties and Responsibilities:

Operations Oversight:

  • Provides daily oversight of Human Resources operations for all program and service areas.
  • Provides support to Managers and Directors aligning with CADES practice, policy, and best human resources practices.
  • Manages communication internally and externally related to all functions of human resources.
  • Provides oversight to the payroll process. Oversight includes support to the Payroll Manager, review of payroll prior to submission and ensuring alignment with applicable local, state, and federal payroll requirements.
  • Responsible for daily operations related to Union employees including ongoing communication with Union Representative and union dues.
  • Provides direction and support to recruitment efforts including management of vacancy list and master schedules.

Risk Management:

  • Coordinates with Quality Assurance Assistant Director to ensure management of high-level work-related injuries.
  • Responsible for investigation of all employee-related concerns reported by employee and/or Manager/Supervisor.
  • Determines outcome for investigations ensuring alignment with regulations, CADES practice, policy, and best HR practices.
  • Supports Manager/Supervisor in determination and delivery of employee performance feedback.

  • Responsible for enrollment through Paycom Benefits Management for all benefit plans.
  • Manages annual Open Enrollment process.
  • Responsible for monthly review and reconciliation of all benefit invoices.

Position Title: Healthcare Supervisor 

FLSA Classification:    Exempt

SUMMARY:

The Healthcare Supervisor is a person, who by the virtue of special knowledge, training, and experience, is uniquely able to inform, motivate, guide, assist, and care for individuals with intellectual/developmental disabilities (I/DD).  In this process, it is the primary responsibility of the Healthcare Supervisor to recognize which problems are beyond the scope of his/her training, skill, or competency and to be willing and able to refer the individual to other appropriate professional services.

The tasks which the Healthcare Supervisor performs and the areas in which he/she is expected to be competent will generally fall into the following categories:

  • Staff education/Training
  • Direct care
  • Supervision of staff
  • Assessment
  • Individualized Service or Education Plans (ISPs or IEPs)
  • Consumer education/Training
  • Service coordination/Case Management
  • Quality Assurance

Duties and Responsibilities:

  • Perform assessments of client’s health status on admission/discharge as well as on an ongoing basis when medical needs indicate and make decisions and/or recommendations for external medical attention contingent upon the circumstance which is then supported by timely point of care (POC) documentation
  • Completes required documentation, specifically:
    • Monthly SBAR report
    • Daily SBAR report
    • Medical appointment paperwork
    • Monthly head to toe assessment
    • Bi-monthly medication reconciliation
    • Pertinent Plans of Care
    • Maintains accurate list of immunizations
  • Communicate and coordinate health information between CADES, Program Directors, and direct care professionals (DSPs), outside provider agencies, Director of Clinical Services, and other health professionals, routinely and whenever further assessment, evaluation or monitoring is required.
  • Screening of individuals for illness/injury and determine the need for the implementation of routine, urgent, and emergency care as identified in CADES Policy and Procedures.
  • Individual case management of assigned individuals which includes coordinating, managing, and monitoring behavioral/medical/dental care and follow up. Assuring that medical/dental appointments are scheduled, and medically necessary information accompanies the individual to the appointments and physician’s orders are implemented.
  • Development and maintenance of heath management plans, records, lifetime medical histories and medication regimes for each individual.
  • Prepare and administer medications, treatments and tasks as ordered and in accordance with CADES Policies and Procedures and the Pennsylvania 6400 Regulations.
  • Evaluate the therapeutic effectiveness of medication with pertinent data and report any observed side effects or relevance laboratory values to physician.
  • Monitor and oversee medication administration/issues for individuals to ensure accurate administration, documentation, and storage.
  • Review monthly medication sheets, clarify need for corrections, make corrections as necessary as well as communicate changes to DSPs, Director of Clinical Services and Program Managers.
  • Ensure PA Nurse Practice Act when obtaining, transcribing, checking, administering, and storing medications; to include medication orders.
  • Communication and coordination of pertinent health concerns or information on a routine basis with the Residential Program Director, Director of Clinical Services, and other CLA Healthcare Supervisors.
  • Attendance at routinely scheduled CLA Nursing Meetings, in-service meetings, and other meetings as necessary.
  • Maintains current knowledge of Emergency Medical Plans for caseloads.
  • Telephone communication is documented on an organizational call log.
  • Reviews all medical visit and discharge reports, ensures appropriate follow-up actions are completed in alignment with all recommendations. Provides feedback to primary care physician on recommendations from discharge summaries for follow-up.
  • Provides health information by instructing residential team in care, rehabilitation of patient; maintaining health and prevention of disease; providing instructions in other subjects related to individual and community welfare.
  • Maintains health care records in alignment with PA Department of Human Services regulations (Chapter 6400) and Medicaid Waiver regulations (Chapter 51).
  • Collaborate in the development and implementation of client annual assessment, health screening tools (such as HRST), ISP health plans and Plans of Care.  Updates electronic health record(s) for any change in status throughout the ISP year. 
  • Coordinates the provision and maintenance of DME, disposable DME and supplements for clients on caseload in conjunction with residential staff.
  • Communicates with client’s family members and designated health care decision makers.
  • Develops individualized plans of care (POCs) and educates residential staff.
  • Provides on-call coverage for the CLA program according to the rotation procedure.

Required Knowledge, Skills and Abilities:

  • LLPN required; RN preferred. Current license in the state of Pennsylvania.
  • Basic lifesaving (BLS) certification
  • Minimum of five years’ experience working with and coordinating healthcare for individuals with multiple disabilities.
  • Current PA State Police & Child Abuse clearances; FBI clearance for anyone living outside PA in the past two years.
  • Valid driver’s license and 3-year adequate driving record; maintain adequate vehicle liability insurance.
