Job Title: Direct Support Professional
Program: Adult Day Program
Location: 401 Rutgers Avenue, Swarthmore PA 19081
Schedule:   Full-Time Floaters – 8:00am to 3:30pm – Monday – Friday – Position includes paid benefits and accrued time off.  On-Call positions also available.
Position Description: CADES is seeking enthusiastic direct support staff to work within our Adult Day Program.  Position works with adults ages 21 and older with intellectual and physical disabilities.  Staff provide necessary services to implement individual support plans and provide personal care needs for consumers.  EOE.

Minimum Requirements:

  • Valid Driver’s License for state of residence with a clean driving record and minimum three year driving history
  • High School diploma or GED
  • Flexibility and reliability
  • Must be able to lift a minimum of 20 pounds, bend, twist and turn
  • Must be able to accomplish personal care duties for adult individuals such as feeding, toileting, diapering and dressing

Program: Adult Day Program

Location:401 Rutgers Avenue, Swarthmore PA 19081

Schedule: Full-time – Monday through Friday – 40 hours

Position Description: CADES is currently seeking a full-time Program Specialist to work in our Adult Day Program in Delaware County. The Program Specialist will oversee and ensure completion of all required program planning for individuals while monitoring and promoting the health and safety of all CADES individuals.

Responsibilities for this position include:

  • Completion & oversight of the implementation of all program planning within the guidelines of regulatory standards: ISPs; monthlies; quarterlies; plans of support.
  • Role model effective communication & problem-solving skills.
  • Develops, with the assistance of Direct Support Professional team active programming that meets the interest of the individuals served and aligns with individual ISP/BSPs, Everyday Lives, Community Rule and CADES vision.
  • Oversee & manage regulatory monitoring for individuals.
  • Monitor & ensure programs are meeting the programmatic, health care, supervision and dietary needs of the individual.
  • Liaison for communicating & updating the team on the needs of the individuals.
  • Act as liaison for external stakeholders (Supports Coordinator, Family, Provider) and internal stakeholders including Nurse, Trainer, Director & Assistant Director for all individuals under assigned areas of oversight.
  • Complete audits & ensure compliance of health and safety of individuals.
  • Actively participate in the Incident Management process.
