Job Title: Community Support Professional

Program: Adult Day Program

Schedule: Full-Time Position – Monday through Friday 8:15am to 3:45pm.

Summary: Community Support Professionals work directly with other team members to implement support plans for adults who have intellectual and/or physical disabilities. CSP’s provide personal care, assistance, and guidance to enhance independence and self-worth for the individuals, we serve both at our center(s) as well as through a wide variety of community experiences.

Responsibilities:

Carry out with thoroughness and accuracy appropriate tasks responsibilities, individual or group activities as designated by the Program to support community participation.

  • Carry out all activities that meet the needs of the individuals we serve through CADES Adult Day Program to ensure alignment with individual ISPs/BSPs, Everyday Lives, Community Rule, and CADES Mission & Vision.
  • Ensure active programming that best meets the interests and needs of the individuals served.
  • Transportation/Community: Provide transportation, supervision, and support during community outings to facilitate active engagement and independence skill-building. 
  • Toileting: Assist individuals in toileting and hygiene as assigned to ensure personal care needs are met for the individuals you are assigned each day. 
  • Feeding: Assist with feeding, meal preparation, and clean-up. Ensure assigned individuals are provided dietary requirements that align with ISP and Plans of Care.
  • Activities: Implement daily activity schedules and provide assistance as outlined in each ISP to maximize independence. 
  • ISP Outcomes and Documentation: Assist each individual on his/her goal work per the ISP. Ensure accurately and timely documentation.
  • Paperwork: Complete all necessary paperwork as assigned, including attendance, incident reports, seizure charts, behavior charts, tube feeding charts, work orders, employee injury reports, and any other assigned paperwork. Complete all paperwork accurately and in a timely fashion.
  • Medication: Obtain/maintain medication administration certification and administer medications according to the procedures when the point person is absent.

Qualified candidates must have:

  • Valid Driver’s License for the state of residence
  • High School diploma or GED
  • Flexibility and reliability
  • Must be able to lift a minimum of 20 pounds, bend, twist, and turn
  • Must be able to accomplish personal care duties for adult individuals such as feeding, toileting, and dressing

Job Title: Program Coordinator

Program: Adult Day Program

Schedule: Full-time – Monday through Friday – 40 hours

Position Description: The Program Coordinator is a full-time exempt position with the purpose/s to implement and supervise the delivery of quality services to individuals participating in the Adult Day Program. The Program Coordinator oversees and ensures the implementation of all ISPs in an environment that fosters independence and promotes programming, that aligns with CADES mission and vision which is closely tied to the philosophy of Everyday Lives for the adults served. Responsibility to ensure compliance with all CADES policies and procedures, state regulations, accrediting measures, and insurance standards is key to this role.

Responsibilities:

  • Align CADES programs and services with mission, vision, and regulatory requirements that positions CADES to be a role model in the delivery of adult day care services to individuals with multiple disabilities and abilities.
  • Oversight of Community Support Professional implementation of all program planning within the guidelines of regulatory standards including assessments, ISPs, monthlies, quarterlies, and plans of support.
  • Develop active programming that meets the interests of the individuals served and aligns with individual ISP/BSPs, Everyday Lives, Community Rule, and CADES vision.
  • Role model effective communication and problem-solving skills with all stakeholders.
  • Active participation in the Incident Management process including; entering incidents into EIM, point person for incidents, timely reporting to appropriate entities (APS), ensure plans of correction for all incidents.
  • Oversee and manage regulatory monitoring for individuals, including data collection, analysis, response to concerns, and implementation of preventative measures.
  • Act as the main liaison for external stakeholders (Supports Coordinator, Family, providers) and internal stakeholders.
  • Provide timely reporting of all concerns to mitigate risk.
  • Liaison for communicating and updating direct care staff on the needs of the individuals.
  • Train, monitor & ensure programs are meeting the programmatic, health care, supervision, and dietary needs of all individuals. Ensures safety in both facility and community settings.
  • Provide ongoing performance feedback to direct care staff to ensure compliance with all ISP, POC, BSP, and guidelines for community participation.
  • Support staff development through performance measures that provide clear and consistent expectations that align with specific duties and responsibilities.

Required Knowledge/Qualifications:

  • Bachelors degree with two years experience working directly with individuals with disabilities or; Associates degree or equivalent with four years experience working directly with individuals with disabilities.
  • Current PA State Police & Child Abuse clearances; FBI clearance for anyone living outside PA in the past two years.
  • Valid driver’s license and 3-year adequate driving record; maintain adequate vehicle liability insurance.
  • Knowledge of PA Department of Public Welfare Chapters 2380 and 11 regulations.
